📱Remote Patient Monitoring Program 📱

Through this program, Health Workers will digitally connect with you when you are experiencing COVID-19 symptoms.

Patients will receive daily virtual visits and receive wellness kits that include things like thermometers, pulse oximeters, blood pressure cuffs and alcohol wipes.

This program serves to reduce barriers to physical health and mental health services in all communities and income levels.

It's available right now in Southern Illinois and Central Illinois, and will be available throughout Northern Illinois this coming week.

For East Central and North Central Illinois communities, call the OSF Healthcare hotline at 1-833-673-5669.
